Association between receipt of guideline-concordant lung cancer treatment and individual- and area-level factors: A spatio-temporal analysis

Wah W, Stirling RG, Ahern S, et al. - In patients with lung cancer, researchers investigated if differences in receipt of guideline-concordant treatment (GCT) was due to clinical features vs contextual factors, including socioeconomic inequalities. Using the National Comprehensive Cancer Network guidelines, GCT receipt was defined. Overall 4,854 non–small cell lung cancer (NSCLC) and small cell lung cancer (SCLC) cases were analyzed. GCT was received by approximately 60.36% of patients. The rates differed across geographic regions over time. The factors mainly related to this variation were poor performance status, advanced clinical stages, NSCLC types, public hospital insurance, area-level deprivation, and comorbidities. Overall, in patients having lung cancer with poor performance status, NSCLC, advanced clinical stage, stage I–III SCLC, stage III NSCLC, public hospital insurance, and comorbidities, and residing in socioeconomically disadvantaged regions, the differences in receipt of GCT was emphasized in this study and must be addressed.
