Assessment of the frequency of dual allergy to penicillins and cefazolin: A systematic review and meta-analysis

Sousa-Pinto B, Blumenthal KG, Courtney L, et al. - Cefazolin (a safer and more efficacious option than second-line alternatives) is less frequently used as prophylactic treatment in surgical patients labeled as having penicillin allergy, which results in higher rates of surgical site infections. Researchers herein investigated the incidence of dual allergy to cefazolin and natural penicillins. Searching MEDLINE/PubMed, Web of Science, and Embase, they identified 77 unique studies meeting the eligibility criteria, yielding 6,147 patients. Forty-four participants with a history of penicillin allergy were identified to have cefazolin allergy, resulting in a dual allergy meta-analytical frequency of 0.7%. Given the low frequency of penicillin-cefazolin dual allergy, they support providing cefazolin to most patients regardless of penicillin allergy history.
