Assessment of the frequency of dual allergy to penicillins and cefazolin: A systematic review and meta-analysis

Prophylactic use of cefazolin is less frequent among surgical patients labeled as having penicillin allergy and hence, these patients more frequently receive clindamycin or vancomycin, which results in higher rates of surgical site infections. Researchers aimed at determining the incidence of dual allergy to cefazolin and natural penicillins via performing a systematic review and meta-analysis. Eligibility criteria were met by 77 unique studies, yielding 6,147 patients. Per findings, there were 44 patients who were allergic to a natural penicillin as well as cefazolin, resulting in a meta-analytical frequency of 0.7%. In view of the low frequency of penicillin-cefazolin dual allergy, they emphasize providing cefazolin to most patients regardless of penicillin allergy history.
