Assessment of renal function before contrast media injection: Right decisions based on inaccurate estimates

Ptáčník V, et al. - To determine the discrepancy between good diagnostic accuracy and poor prediction of glomerular filtration rate (GFR) estimated by prediction equations, researchers studied 50 patients for GFR, measured as plasma clearance of 51-chromium labeled ethylene-diamine-tetraacetic-acid (51Cr-EDTA) and compared with GFR, calculated by creatinine clearance (CC). Most patient's GFR came out below specified levels, even after large variance and poor prediction accuracy of individual GFR estimates. If needed, more accurate radionuclide or laboratory methods should be considered to calculate the numerical values of GFR, while usage and reporting of prediction equations should be the exclusive binary variables.
