Assessment of postoperative healing following endoscopic, transnasal, transsphenoidal pituitary surgery without formal sellar grafting

Jandali D, et al. - In this single institution retrospective chart review involving 83 patients, researchers evaluated healing and mucosalization of the sellar face following transsphenoidal surgery (TSS) without formal sellar grafting or reconstruction. Participants in the study were patients undergoing TSS without intraoperative CSF leaks between January 2014 and March 2017 at Rush University Medical Center. Data reported that mean time to mucosalization was 119 days (range, 17 to 402 days). The authors discovered that the incidence of abnormal mucosal healing, epistaxis, crusting and scarring rose from the first visit to the second postoperative visit but the third visit trended down. The most common finding was nasal crusting, followed by abnormal mucosal healing. Without formal grafting or reconstruction after TSS, adequate sellar healing is achievable in all instances. Due to the small inherent risk of unmasking a subclinical intraoperative CSF leak, great care must be exercised. In order to minimize the impact of crusting, patients should be closely followed endoscopically during the first four months after TSS.
