Are sleep studies necessary prior to adenotonsillectomy? A retrospective case controlled study of eighty-two children

Elphick HE, et al. - Given delays and cost related to overnight pulse oximetry, performed to determine severity of sleep apnoea and the possibility of post-operative complications, researchers sought the necessity for sleep studies prior to adenotonsillectomy. Among 82 children without co morbidities, 15 (36.6%) exhibited mild post-operative respiratory complications requiring repositioning or oxygen in those with severely abnormal oximetry (SpO2 4% dip index > 10/hr and/or SpO2 nadir <80%) and 4 (9.8%) in matched controls. In hospitals without high dependency care (HDU) facilities, they recommend performing overnight oximetry and transferring children with severely abnormal studies to a centre with HDU. Safe management in other cases could be done with day-surgery with admission if there is a significant oxygen requirement after 6 hours.
