Aortic stenosis with severe tricuspid regurgitation: Comparative study between conservative transcatheter aortic valve replacement and surgical aortic valve replacement combined with tricuspid repair

Rozenbaum Z, et al. - In this nonrandomized, retrospective, observational study, researchers determined the change in right ventricular (RV) function and tricuspid regurgitation (TR) after surgical aortic valve replacement combined with tricuspid valve repair (SAVR+TVr), transcatheter aortic valve replacement (TAVR), or conservative management and compared outcomes dependent on RV functional parameters and treatment allocation. In patients with severe aortic stenosis (AS) combined with severe TR, SAVR+TVr and TAVR were linked to reductions in TR. They found that RV function improved after TAVR but not after SAVR+TVr. It was noted that patients with severe AS and TR had a very poor prognosis with conservative therapy. The study findings suggested that assessment should include quantitative functional RV parameters when contemplating invasive procedures.
