Antimicrobial stewardship near the end of life in aged care homes

Dowson L, Friedman ND, Marshall C, et al. - In this study, researchers sought to assess aged care home health professionals' perceptions regarding antimicrobial use near the end of life and potential antimicrobial stewardship activities near the end of life in aged care homes. General practitioners, nurses, and pharmacists, who provide routine care in aged care homes in Victoria, Australia, underwent qualitative semi-structured interviews. The establishment of themes was done within 14 interviews, and to ensure thematic saturation, 6 interviews were undertaken additionally. The emergence of two major themes was noted: (i) Antimicrobial stewardship activities near the end of life in aged care homes are required to allow aged care home nurses to make decisions confirmed by evidence-based clinical knowledge; (ii) Family confidence in resident wellbeing should be addressed in antimicrobial stewardship activities near the end of life in aged care homes. Based on these findings, they emphasize the requirement for antimicrobial stewardship activities that reinforce evidence-based clinical decision-making by aged care home nurses and address family confidence in resident wellbeing near the end of life in aged care homes. Findings suggest accreditation standards as relevant motivators for behavior change in aged care homes.
