Antimicrobial resistance in patients with decompensated liver cirrhosis and bacterial infections in a tertiary center in Northern Germany

Hillert A, Schultalbers M, Tergast TL, et al. - Among patients with liver cirrhosis and infection, the antimicrobial resistance profile of bacteria was determined. Bacterial infection was assessed in overall 666 cirrhotic patients admitted with ascites to Hannover Medical School between January 2012 and April 2018. Microbiological sampling was performed in 281 patients with infection, and culture-positive results were retrieved in 56.9%. They identified multidrug-resistant (MDR)-bacteria in 54 patients (19.2%). Overall, different ratios of gram-positive infection were identified with overall low ratios of MDR-bacteria in this tertiary center. Based on findings, they recommend considering adequate gram-positive coverage in the empiric therapy. Omission of carbapenem treatment may be considered even in nosocomial infection. In contrast, recommendation of 3rd generation cephalosporins cannot be made even in community-acquired infection in this cirrhotic population.
