Anticonvulsant primary and secondary prophylaxis for acute ischemic stroke patients: A decision analysis

Jones FJS, Sanches PR, Smith JR, et al. - Researchers conducted the study for analyzing the impact of 3 anticonvulsant prophylaxis strategies on quality-adjusted life-years among patients with an incident acute ischemic stroke. They created a decision tree to assess 3 strategies: long-term primary prophylaxis; short-term secondary prophylaxis after an early seizure with lifetime prophylaxis if persistent or late seizures developed; and long-term secondary prophylaxis if either early, late, or persistent seizures developed. In all scenarios studied, short-term therapy with continued anticonvulsant prophylaxis only after postischemic stroke seizures occur outperforms lifetime primary prophylaxis. The findings highlight the importance of close monitoring and the discontinuation of anticonvulsant seizure prophylaxis initiated during acute ischemic stroke hospitalization.
