Anti-glomerular basement membrane antibody-mediated glomerulonephritis combined with non-fulminant acute viral hepatitis A

Kim MK, et al. - The researchers reported the first case of the anti-glomerular basement membrane (anti-GBM) antibody-mediated glomerulonephritis (Ab-mediated GN) (rare autoimmune disease, characterized by crescentic glomeruli and linear GBM staining with immunoglobulin G [IgG]) relative to hepatitis A virus (HAV) infection, since exposure to viruses can trigger the formation of autoantibodies to the GBM. However, the pathogenesis of HAV-related anti-GBM Ab-mediated GN has not been noted, it was inferred that the immune complexes created by HAV infection might get deposited in the glomerular capillary walls and result in immune complex-mediated glomerular damage, leading to exposure of the sequestered GBM antigens and the formation of anti-GBM antibodies. The renal function had been getting better without the development of pulmonary symptoms, under immunosuppressive and intravenous cyclophosphamide therapy.
