Antenatal corticosteroid administration and early school age child development: A regression discontinuity study in british columbia, canada

Hutcheon JA, Harper S, Liauw J, et al. - In view of increasing concerns that antenatal corticosteroid administration may harm children’s neurodevelopment, researchers sought to ascertain the safety of antenatal corticosteroid administration practices for children’s overall developmental health (skills and behaviors) at early school age. Population health and education databases from British Columbia (BC), Canada were linked to obtain a cohort of births admitted to hospital between 31 weeks, 0 days gestation (31+0 weeks), and 36+6 weeks, 2000 to 2013, with routine early school age child development testing. During the studied time period, antenatal corticosteroids were administered to most women admitted for delivery before 34 weeks of pregnancy, but not to most women admitted after 34 weeks. Except for their chance of receiving antenatal corticosteroids, pregnancies admitted for delivery just before and just after 34 weeks are similar in most ways. Hence, researchers herein compared children whose mothers were admitted just before and just after 34 weeks of pregnancy regarding their development test scores in order to identify if children who would have been exposed to antenatal corticosteroids according to clinical practice guidelines attain lower scores. Findings revealed no difference in child development test scores between children delivered just before and just after 34 weeks of pregnancy, thereby suggesting no negative impact of a policy for routine administration of antenatal corticosteroids on overall child developmental health.
