Aneurysm wall enhancement in unruptured intracranial aneurysms: A histopathological evaluation

Zhong W, Su W, Li T, et al. - Researchers here examined the correlation between aneurysm wall enhancement (AWE) and histopathological changes via this histopathological evaluation that prospectively enrolled a total of 35 patients with 41 unruptured intracerebral aneurysms who underwent surgical clipping. For evaluation, a total of 27 aneurysms were available. AWE was identified in a total of 21 (51.2%) aneurysms (partial AWE, n = 7; circumferential AWE, n = 14). On univariate analysis, aneurysm size, irregularity, and atherosclerotic and translucent aneurysms were associated with AWE, while atherosclerosis was the only factor significantly and independently associated with AWE in multivariate logistic regression analysis. Observations revealed that despite a possible correlation of AWE on vessel wall magnetic resonance imaging scans with the presence of atherosclerotic lesions in unruptured intracerebral aneurysms, inflammatory cell infiltration within atherosclerosis, intraluminal thrombus, and vasa vasorum may represent the primary pathological features linked with AWE.
