Anesthetic approach to postdural puncture headache in the peripartum period: An Israeli national survey

Ioscovich A, et al. - Researchers surveyed the lead anesthesiologist in 23 labor and delivery rooms in Israel to gain information on different aspects of treatment and post-dural puncture headache (PDPH) management strategies in the peripartum period. They interrogated the participants of the survey to obtain information regarding: medical center annual delivery volume, training program for residents, accidental dural puncture management, processing of information, and PDPH management strategies. The gold standard treatment for PDPH was epidural blood patch (EBP), although inefficacy of prophylactic blood patch was reported. In most centers, a tendency of very low performance of both prophylactic EBP and epidural normal saline administration after delivery was documented. In most hospitals, 24-48 hours of conservative treatment was followed by performing EBP, along with published recommendations that demonstrated increased EBP efficiency with this timeframe.
