An outbreak of severe Kawasaki-like disease at the Italian epicentre of the SARS-CoV-2 epidemic: An observational cohort study

Verdoni L, Mazza A, Gervasoni A, et al. - Researchers conducted this observational cohort study to characterize the incidence and features of new cases of Kawasaki-like presentations admitted to the unit during the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) epidemic. All patients diagnosed with a Kawasaki-like disease at the centre over the past 5 years have been divided by symptomatic presentation before (group 1) or after (group 2) the onset of the SARS-CoV-2 epidemic. Group 1 consisted of 19 patients (aged 3·0 years [SD 2·5]) diagnosed between January 1, 2015, and February 17, 2020. Group 2 involved ten patients (aged 7·5 years [SD 3·5]) diagnosed between February 18 and April 20, 2020; eight out of ten were IgG or IgM positive, or both. The authors observed a 30-fold increased incidence of Kawasaki-like disease in the past month. Children diagnosed at the beginning of the SARS-CoV-2 epidemic showed signs of immune response to the virus, were older, had a higher rate of cardiac involvement, and macrophage activation syndrome features. The epidemic of SARS-CoV-2 was associated with a high incidence of a severe form of Kawasaki disease. In countries participating in the SARS-CoV-2 epidemic, a similar outbreak of Kawasaki-like disease is expected.
