All-cause mortality and serious non-AIDS events in adults with low-level HIV viremia during combination antiretroviral therapy: Results from a Swedish nationwide observational study

Elvstam O, Marrone G, Medstrand P, et al. - Researchers examined if and how low-level viremia (LLV) is associated with all-cause mortality, AIDS, and serious non-AIDS events (SNAE). They followed 6,956 participants (identified from the Swedish InfCare HIV register) for a median of 5.7 years and grouped them as virologic suppression (VS; < 50 copies/mL), LLV (repeated viral load 50–999 copies/mL), and non-suppressed viremia (NSV; ≥ 1.000 copies/mL). Observations revealed association of LLV during combination antiretroviral therapy with adverse clinical outcomes in this population-based cohort. LLV was linked with increased mortality when compared with VS. For LLV 50–199 copies/mL, but not for LLV 200–999 copies/mL, this association was statistically significant. No association of LLV 50–999 copies/mL with increased risk of AIDS or SNAE was observed, but in subanalysis, LLV 200–999 copies/mL was linked with SNAE.
