Age-dependent association of occult hypoperfusion and outcome in trauma

Hatton GE, McNutt MK, Cotton BA, et al. - Given the risk of poor outcomes in elderly trauma patients in correlation with occult hypoperfusion (OH), or global hypoperfusion with normal vital signs, researchers here examined if OH is correlated with worse outcomes than shock in both young and elderly trauma patients. In this single-center cohort study, they included 3,126 adult (≥ 16 years) trauma patients; of these 808 were elderly. Young and elderly patients were similar regarding rates of shock (33% and 31%) and OH (25% and 23%). The analysis revealed that OH is associated with worse outcomes than shock among the elderly, but not among younger patients. They emphasize to pay more attention to the diagnosis and treatment of all hypoperfused states in the elderly although redefinition of shock parameters seemed required in this age group.
