Adverse outcomes and short-term cost implications of bile duct injury during cholecystectomy

O’Brien S, et al. - Researchers examined patients undergoing cholecystectomy for the healthcare costs, length of stay, and discharge status associated with bile duct injury in the United States. Querying Premier Healthcare Database, which comprises hospital-billing records from over 700 hospitals in the United States, they identified a total of 1,168,288 cholecystectomies with bile duct injury (BDI) in 878 patients (0.08%). The most common approach was laparoscopy (> 95%). Observations revealed significantly increased hospital costs ($18,771 vs $12,345), length of stay (8.6 days vs 4.8 days), 30-day readmission (odds ratio 1.86), and discharge to an institutional post-acute care facility (odds ratio 3.89) among cases with BDI vs no BDI. During inpatient admissions, the occurrence of the majority of BDI (71.0%) was reported.
