Adverse events in long-term care residents transitioning from hospital back to nursing home

Kapoor A, et al. - In this prospective cohort study, researchers reported the incidence, type, severity, and preventability of adverse events in a sample of long-term care (LTC) residents transitioning from hospital back to the same LTC facility. A total of 762 hospital discharges were identified for 555 individual LTC residents. Investigators found that adverse events occurred in almost 4 out of 10 hospital back to LTC discharges. Most of them were preventable or ameliorable. Standardized event reporting and improved coordination and information transfer across settings may be methods of preventing adverse events in returning LTC residents. The most frequent types of adverse events were resident care-related adverse events, including falls with injury, pressure ulcers, and skin tears.
