Adverse effect of laparoscopic radical hysterectomy depends on tumor size in patients with cervical cancer

Hu TWY, et al. - In this retrospective study, researchers examined women who had undergone laparoscopic/abdominal radical hysterectomy (LRH/ARH) for early-stage cervical cancer (CC) in West China Second University Hospital, to determine the survival outcomes in these patients. Participants were 678 in total. No significant differences were found between the ARH (n = 423) and LRH (n = 255) groups with regard to overall survival (OS) and progression-free survival (PFS), however, patients with a tumor diameter > 4 cm in the LRH group had significantly shorter OS vs ARH group. In multivariate Cox analysis, independent prognostic factors for OS and PFS were revealed: International Federation of Gynecology and Obstetrics stage, histology, parametrial invasion, and pelvic lymph node invasion. OS or PFS in CC patients was not statistically significantly predicted by surgical method. For CC patients with a tumor diameter ≤ 4 cm, LRH was identified as an alternative to ARH for operative management. However, giving priority to ARH for those with a tumor diameter > 4 cm was recommended.
