Advancing early identification of axial spondyloarthritis: An interobserver comparison of extended role practitioners and rheumatologists

Passalent L, Hawke C, Lawson DO, et al. - In this research, clinical impression and confidence of extended role practitioners (ERP) were compared with those of rheumatologists experienced in axial spondyloarthritis (axSpA) according to investigation of patients with chronic back pain assessed for axSpA and MRI recommendation for further evaluation of these patients. There were 57 individuals who were included in this study. For axSpA assessment, individuals with ≥ 3 months of back pain and age of onset < 45 years were referred. Researchers divided individuals as having axSpA or mechanical back pain based on clinical and investigative findings. They noted level of confidence for categorization and MRI indication. Percentage agreement and κ coefficient were applied to test agreement between assessors. For the evaluation of axSpA, they findings exhibit that ERP with particularly training in inflammatory arthritis show clinical impressions comparable with those of rheumatologists. The evidence suggested that incorporation of such roles into existing models of care may assist in early detection of axSpA.
