Adoption of total neoadjuvant therapy for locally advanced rectal cancer

Cercek A, et al. - The traditional approach of preoperative chemoradiation (chemoRT) followed by postoperative adjuvant chemotherapy was compared with the more recent total neoadjuvant therapy (TNT) approach for locally advanced rectal (LARC) cancer. Researchers noted improved delivery of systemic therapy and increased response to treatment in association with total neoadjuvant therapy. In addition, TNT seemed to provide a promising platform for nonoperative watch-and-wait protocols. They suggested the necessity for long-term follow-up to determine if early systemic chemotherapy improves overall outcome.
