Adenocarcinoma of the oesophagogastric junction Siewert II: An oesophageal cancer better cured with total gastrectomy

Voron T, et al. - Researchers examined 183 patients with type II adenocarcinoma of the oesophagogastric junction (AEG) treated from 1997 to 2010 in 21 French centers by superior polar oesogastrectomy (SPO) or total gastrectomy (TG), for their surgical and oncological outcomes. Sixty-four (35%) patients underwent a TG whereas SPO was performed on 119 (65%) patients with transthoracic approach in 100 of them (83.2%) and transhiatal approach with cervicotomy in 19 (16.8%). The two approaches led to comparable surgical outcomes. Patients operated on by TG vs SPO had median survival of 46 months vs 27 months, respectively. Multivariate analysis suggested independent good prognostic value of TG vs SPO. However, they observed a higher rate of incomplete resection in correlation to TG. As per findings, they recommend preferring TG to SPO when TG allows obtaining tumor-free resection margins.
