Acute kidney injury after out of hospital pediatric cardiac arrest

Cornell TT, et al. - In comatose children, the prevalence of severe acute kidney injury (AKI) after out-of-hospital cardiac arrest (OHCA) and the associated risk factors were evaluated. In addition, the influence of therapeutic hypothermia on severe AKI prevalence and its association with survival and functional outcomes were investigated. They performed a post hoc secondary analysis of data from the Therapeutic Hypothermia after Pediatric Cardiac Arrest Out-of-Hospital (THAPCA-OH) trial at 36 pediatric intensive care units in the US and Canada. Researchers encountered severe AKI frequently in children with ROSC after OHCA, especially in younger children and those with higher initial lactates and hemodynamic support. Worse survival and functional outcome were linked to severe AKI. Its incidence was not reduced with therapeutic hypothermia.
