Accuracy of patient self-administered medication history forms in the Emergency Department

Wai A, et al. - Conducting a cross-sectional study in an urban Emergency Department (ED) in Australia, researchers primarily assessed the proportion of patients with medication discrepancies when using a self-administered medication history form in the ED. In this work, identification of predictors of medication discrepancies and determination of the proportion of patients with a high-risk medication discrepancy were included as secondary objectives. They analyzed a total of 138 patients and identified a high rate of discrepancies in patient self-administered medication history forms. They recommend confirming this by the best possible medication history. Rate of discrepancies was noted to be significantly correlated with the number of medications, female, and missing community pharmacy information.
