Accelerated venovenous hemofiltration as a transitional renal replacement therapy in the intensive care unit

Allegretti AS, Endres P, Parris T, et al. - Researchers assessed accelerated venovenous hemofiltration (AVVH) as a transition therapy between continuous renal replacement therapy (CRRT) and intermittent hemodialysis (iHD), with the goal of reducing time on CRRT and affording a more graduated transition for hemodynamically unstable patients needing RRT. In this retrospective cohort analysis at an academic tertiary care center, they evaluated the clinical results as well as quality initiative experience of the integration of AVVH into the CRRT program. Overall 298 AVVH treatments were administered to 97 patients. Successful completion of 271/298 (91%) treatments was reported. Findings highlighted the potential of AVVH in providing reduced total time on CRRT and improved patient mobility, as well as low ICU readmission rates. AVVH can be used as a transition therapy between CRRT and iHD in the ICU.
