Absence of prognostic value of lymphovascular space invasion in patients with endometrial cancer and negative sentinel lymph nodes

Matanes E, Eisenberg N, Lau S, et al. - Researchers examined the prognostic value of lymphovascular space invasion (LVSI) in endometrial cancer patients with negative lymph nodes following sentinel lymph node (SLN) mapping vs lymph node dissection (LND) as staging procedure. They conducted a retrospective study of 510 patients diagnosed with endometrial carcinoma identifying 413 patients who met the inclusion criteria for this study; Of these 413 patients, 239 underwent SLN and 174 patients underwent LND only. Findings revealed a high association of LVSI with nodal metastases in endometrial cancer. When patients undergo SLN mapping and are identified to have negative nodes, the prognostic value of LVSI disappears in contrast to patients who have undergone LND. Sentinel node mapping could modify the value of “soft” risk factors.
