Abdominal radical trachelectomy vs chemotherapy followed by vaginal radical trachelectomy in stage 1B2 (FIGO 2018) cervical cancer. A systematic review on fertility and recurrence rates

van Kol KG, et al. - Via a systematic review of ten studies with a total of 338 individuals, researchers contrasted rates of fertility, pregnancy, live births as well as a recurrence for females with cervical cancer stage IB2 treated with either neoadjuvant platinum-based chemotherapy (NACT) followed by vaginal radical trachelectomy (VRT), or abdominal radical trachelectomy (ART). Subsequent to NACT followed by VRT 39% of the women attempted to conceive, 70% of these women conceived, of which 63% of them gave live birth. The overall recurrence and death rates, respectively, were 10% and 2.9%. Following ART 40% of the women who attempted to conceive, 21% of these women got pregnant, which led to a live birth rate of 42%. Recurrence and death rates following ART, respectively, were 6.9%, and 3.4%. Hence, in females with cervical cancer stage IB2 and a wish to preserve fertility, a significantly higher possibility of pregnancy was seen when treated with NACT followed by VRT vs treated with ART, with similar oncological outcomes.
