A validated risk stratification tool for detecting high-risk small bowel Crohn disease

Shen EX, Lord A, Doecke JD, et al. - Three cohorts were acquired for this study (cohort 1, n = 61 individuals, 78 controls; cohort 2, n = 42 persons, 57 controls; cohort 3, n = 84 patients and 495 controls) in order to make a simple risk evaluation tool to facilitate primary care physicians to identify possible ileal Crohn disease (CD) earlier, decreasing the impediment to specialist investigation. Significant characteristics in prognosticating ileal CD were altered bowel habit with abdominal pain combined with derangements in white cell count, albumin and platelet counts. This was confirmed in cohorts 2 and 3. CRP was individually related to the ileal CD yet unimportant in a multivariate model. Thus, from objective and symptom-based criteria, a web-based risk stratification tool for ileal CD has been created. For individuals classified as a high risk for ileal CD, this tool allows primary care physicians to more assuredly request an urgent specialist evaluation.
