A systematic review of paraneoplastic syndromes associated with thymoma: Treatment modalities, recurrence, and outcomes in resected cases

Zhao J, Bhatnagar V, Ding L, et al. - Researchers examined the correlation between surgically resected thymomas and paraneoplastic syndromes (PNS) over the past half-century. Searching PubMed/MEDLINE, they identified 407 articles reporting 507 patients from 1960-2019; of these, 123 different PNS were correlated with thymoma. The following five PNS were most commonly encountered: myasthenia gravis, pure red cell aplasia, lichen planus, Good syndrome, and limbic encephalitis. Seventy-six percent of patients showed complete or partial resolution of PNS symptoms following surgery; of these, 21% experienced a relapse or new PNS onset after surgery. The most common adjunctive treatment linked with resolution of PNS was corticosteroids (30%). These findings suggest that PNS resolution in a majority of patients could be achieved via undertaking a multimodal treatment approach that includes surgical resection of thymomas. The onset of new PNS after surgery is linked with the recurrence of the first PNS and increased overall survival was observed in correlation to the resolution of PNS.
