A retrospective analysis of 1,717 paranasal sinus fungus ball cases from 2008 to 2017

Liu X, Liu C, Wei H, et al. - In the last 10 years in the Department of Otolaryngology-Head and Neck Surgery, Beijing Tongren Hospital, Capital Medical University, Beijing, China, researchers analyzed the epidemiological features, clinical characteristics, and the results of histopathology and microbiology for fungus ball (FB). Between 2008 and 2017, a total of 1,717 cases of FB were reviewed. A significant increase in patients with FB presenting at the department has occurred in the past 10 years. A total of 1,626 cases were unilateral, with the maxillary sinus being the most common location. The most abundant fungal species in the study was Aspergillus spp. (72.8%). It was noted that the direct smear positive rate was 68.6% and the fungal culture rate was 22.6%. Findings suggested that the diagnosis of FB requires histopathological and microbiological analyses. The most effective treatment method is surgery. There is no need for direct smear after surgery. After surgery or during the perioperative era, oral or topical antimycotic treatments are not advised. All cases have been treated with functional endoscopic sinus surgery, with very low postoperative complications and reoperation rate.
