A retro-prospective study in HIV+ patients affected by lymphoma: The Musthal multicenter experience in Northern Italy

Rindone G, et al. - Researchers collected clinical and immunovirological data in HIV+ patients (pts) affected by non-Hodgkin lymphoma (NHL) in this retrospective and prospective multicentre study to delineate the biopathological features of a cohort of HIV+ pts affected by lymphoma and their clinical implications. They enrolled 127 pts (Median age: 48.9 years) in this study: 86 DLBCL, 18 Burkitt lymphomas (BL), 6 primary central nervous system lymphomas (PCNSL), 11 plasmablastic lymphomas (PBL), 3 primary effusive lymphomas (PEL) and 3 anaplastic large T-cell lymphoma (ALCL). Of these, 30% were on cART at NHL diagnosis. R-CHOP and R-DA-EPOCH or more intensive regimens were provided to 80/127pts and 40/127 pts, respectively. A worse survival was observed in correlation to age, high IPI score and CD4+ < 200/μl at diagnosis. This cohort of HIV+ pts usually had reliable lymphoma prognostic scores. They noted an independent influence of CD4+ at diagnosis on NHL survival. They emphasize standardizing biopathological analyses to find rapidly HGBL-affected pts also in HIV+ population, as the response to chemotherapy seems to be poor. Death was reported for the majority of pts who progressed after first line of therapy without receiving autologous bone marrow transplant. The possibility to create new studies and treatment protocols in these pts is supported by the findings.
