A randomized controlled trial comparing epidural analgesia vs continuous local anesthetic infiltration via abdominal wound catheter in open liver resection

Bell R, et al. - Researchers compared outcomes following open liver resection (OLR) between patients receiving thoracic epidural (EP) vs abdominal wound catheters plus patient-controlled analgesia (AWC-PCA) in this randomized controlled trial. They randomized patients 1:1 to either EP or AWC-PCA within an enhanced recovery protocol. Outcomes revealed reduced treatment failure and a reduced vasopressor requirement in correlation with AWC-PCA than EP up to 2 days postoperatively. The use of AWC-PCA did not shorten LOS in this study, however, it simplified patient management after OLR. They do not recommend routine use of EP following open liver resections.
