A randomized control trial of botulinum toxin A administration under ultrasound guidance against manual palpation in spastic cerebral palsy

Kaushik PS, et al. - In children (aged up to 6 years) with spastic cerebral palsy (CP), researchers compared the effectiveness of botulinum toxin when given with ultrasound guidance vs manual palpation. Before and after administration, children were evaluated with range of ankle dorsiflexion, Modified Ashworth Scale (MAS) and Gross Motor Function Measure 66(GMFM 66). Thirty of the 180 children screened were included who met the criteria. Children enrolled in the study were classified in group I and group II, those who were given botulinum toxin with ultrasound (n = 14) and without ultrasound (n = 16), respectively. In terms of the technique of administration of botulinum toxin with ultrasound vs without ultrasound into the gastrocnemius muscle, no significant difference was observed in the outcome. Therefore, ultrasound guidance is not required for the administration of botulinum toxin to gastrocnemius muscle. Overall, they concluded that botulinum toxin is a very effective method for CP spasticity management.
