A randomised controlled trial comparing needle movements during combined spinal-epidural anaesthesia with and without ultrasound assistance

Chin A, et al. - In women undergoing caesarean section with combined spinal-epidural anaesthesia, researchers investigated the efficacy of neuraxial ultrasound and factors associated with improved technical performance. Among a total of 218 women, first-pass success was achieved in 67 (63.8%) and 42 (38.2%) women in the ultrasound and control groups, respectively. Anaesthetists misidentified the intervertebral level by two or more spaces in 23 (10.7%) women. In women with easily palpable spinous processes, ultrasound was shown to be beneficial in sub-group analysis. The use of ultrasound and easily palpable spinous processes were found to be related to first-pass success in regression analysis.
