A prospective randomized comparative study of three guided bronchoscopic approaches for investigating pulmonary nodules (the PRECISION-1 Study)

Yarmus L, Akulian J, Wahidi M, et al. - In a human cadaver model of peripheral pulmonary nodules (PPN) < 2cm, researchers evaluated success in localization as well as puncture of PPN employing an ultrathin bronchoscope with radial endobronchial ultrasound (EBUS) (UTB-rEBUS) vs electromagnetic navigational bronchoscopy (EMN) vs robotic bronchoscopy (RB) in this prospective single blinded randomized controlled comparative investigation. The capability to successfully localize and puncture the target nodule confirmed using cone-beam computed tomography comparing RB and EMN was the primary endpoint. Needle to target position, “miss” distance and UTB-rEBUS comparisons were considered as secondary endpoints. Relative to the existing technologies, RB provided significantly increased ability to localize and successfully puncture small PPN in a cadaver model. As per the findings, RB has the potential to accurately reach, localize and puncture small nodules in the periphery of the lung.
