A practical two-stage frailty assessment for older adults undergoing aortic valve replacement

Hosler QP, et al. - In this prospective cohort study involving patients undergoing aortic valve replacement, researchers compared five brief frailty tests for predicting post-procedural poor outcomes. They also assessed a strategy of performing comprehensive geriatric assessment (CGA) in screen-positive patients. At baseline, they measured brief frailty tests and a deficit-accumulation frailty index based on CGA. At 6 months, a composite of mortality or functional decline and severe symptoms was evaluated. They found that, overall, older patients at high risk for death and poor recovery post-surgical aortic valve replacement and- transcatheter aortic valve replacement—in whom considering individualized care management is recommended—could be identified by the chair rise test with targeted CGA.
