A meta-analysis of surgical vs nonsurgical treatment of primary patella dislocation

Pagliazzi G, et al. - In this analysis, researchers contrasted results in the management of primary lateral patellar dislocation (LPD), defined as an acute patellar luxation from its normal position in the trochlear groove, after surgery vs nonsurgical therapy through a meta-analysis of randomized controlled trials (RCTs) in terms of redislocation frequency and clinical outcome, studying both short-term (< 6 years) functional recovery and overall benefit over time (> 6 years). In November 2018, a systematic search of the literature was performed. In the meta-analysis, 510 patients from 10 RCTs were included. A few high-level trials are documented in the literature. The RCT meta-analysis underlined that the rate of redislocation with the nonsurgical approach is higher than with the surgical approach. In addition, more favorable findings were found in the surgical approach up to 6 years when looking at the clinical outcome, whereas the results appear to be similar in a longer follow-up after either surgical or nonsurgical primary LPD treatment.
