A comparison of pathologic and intermediate term oncologic outcomes following open and robotic radical cystectomy

Kukreja JB, et al. - In a retrospective cohort study at a high volume academic tertiary referral center, researchers focused on recurrence patterns and pathologic comparisons between patients who underwent open radical cystectomy (ORC) vs those who had robotic radical cystectomy (RRC) for bladder cancer from 2005 to 2017. The factors predictive of recurrence free survival (RFS) and overall survival after radical cystectomy (RC) were determined in a multivariable analysis. Overall 1,813 patients were included, with 10% treated with RRC. Findings revealed RRC vs ORC cohorts did not differ in terms of recurrence patterns, severity of pathology distribution, and positive surgical margin status. The factors that showed a link to shorter RFS included younger age, neoadjuvant chemotherapy, higher pathologic stage, positive lymph nodes at RC and positive surgical margins. Overall, RRC vs ORC offered comparable outcomes and was supported as a safe oncologic procedure.
