A comparison of deltopectoral vs Judet approach for glenoid exposure

Ao R, et al. - Researchers recognized the exposure range of the glenoid rim by two approaches (deltopectoral and Judet approaches) and provide reference data for the choice of surgical approach by dissecting 12 cadaveric shoulders. The range of the exposed glenoid rim was from 5:50 to 11:30, which deemed for about 47.2% of the clock face, for the deltopectoral approach whereas the range of the exposed glenoid rim was from 1:30 to 6:20 for the Judet approach, which accounted for about 40.3% of the clock face. An area of partial overlap for the two approaches along the inferior glenoid was found. The superior glenoid rim located from 11:30 to 1:30 was recognized inaccessible since it could not be exposed by the two approaches. Therefore, it was concluded that less than 50% of the glenoid rim can be exposed by the deltopectoral or Judet approach. Moreover, with a single approach, it may be challenging to expose and fix some complex glenoid fractures. Further, the superior part of the glenoid fracture is the non-access area through the deltopectoral or Judet approach.
