A comparison of anterior and posterior lumbar interbody fusions: Complications, readmissions, discharge dispositions, and costs

Qureshi R, et al. - This study entailed comparison between anterior lumbar interbody fusion (ALIFs) vs transforaminal lumbar interbody fusions (TLIFs)/posterior lumbar interbody fusions (PLIFs) for lumbar degenerative disease, focusing on medical complication rates, readmission rates, costs, and discharge dispositions. ALIF procedures were associated with significantly increased readmissions and costs of postoperative care, despite higher initial routine discharge rates. These data call for an assessment of the etiology of degenerative pathology as ALIFs are successful solutions to anterior translational instability and anterior disc slippage, but in comparison to a TLIF/PLIF, ALIFs may not have the best long-term outcomes and may not be cost-effective. Overall, varying approaches should be assessed with respect to the risks and benefits and it is necessary to access the anterior column, when deciding on the surgical technique to treat lumbar degenerative pathology.
