5-Item modified frailty index as a preoperative predictor of morbidity following minimally invasive partial nephrectomy

Goldwag J, et al. - Among patients receiving minimally invasive partial nephrectomy (MIPN), researchers intended to apply the modified frailty index-5 (mFI-5) to assess the link between frailty and postoperative results. They used the ACS-NSQIP database (American College of Surgeons National Surgical Quality Improvement Program) to obtain data for cases coded for MIPN. They found 18,904 cases between 2012 and 2018, of which 38.0% of patients had mFI-5 score of 0, 42.8% had 1, and 19.2% had score of ≥2. With increasing mFI-5 score, a significant increment in multiple postoperative morbidities was noted. For patients exhibiting mFI-5 score ≥2, nearly 50% increase in the odds of having an overall complication was identified in multivariate analysis. Findings revealed that postoperative outcomes in patients undergoing MIPN can be predicted by mFI-5, which affords a well defined and readily ascertainable tool for this purpose. Also, mFI-5 could be employed preoperatively to recognize and counsel patients at elevated risk for morbidity.
