Prediabetes PSAs: The People You Know
More than 84 million American adults have prediabetes. That’s a little hard to wrap your head around. The same fact put another way: “1 in 3 US adults.” It could be you, your favorite brother, or your other brother who has prediabetes. When viewers see themselves and the people they know in the stats, they get it in a whole new way. That’s the thinking behind Phase 3 of CDC’s award-winning Prediabetes Awareness Campaign. Diabetes Self-Management Education and Support (DSMES) Toolkit
Diabetes self-management education and support (DSMES) provides an evidence-based foundation to empower people with diabetes to navigate self-management decisions and activities. DSMES is a cost-effective tool proven to help improve health behaviors and health outcomes for people with diabetes. The purpose of this toolkit is to increase use of DSMES services among people with diabetes and healthcare professionals. Rx for National Diabetes Prevention Program
The Rx for the National Diabetes Prevention Program: Action Guide for Community Pharmacists is designed to help community pharmacists and members of the pharmacy workforce reach people at high risk of developing type 2 diabetes who could benefit from the National Diabetes Prevention Program (National DPP) lifestyle change program and prevent new cases of type 2 diabetes among patients at high risk by taking steps to help expand the reach of the National DPP.
2017 Diabetes Report Card
CDC publishes the Diabetes Report Card every 2 years to provide current information on the state of diabetes and its complications in the nation. Public health professionals, state health departments, and communities can use these data—including information on diabetes, prediabetes, risk factors, and health outcomes—to focus their diabetes prevention and control efforts where they are needed most.
